Private Home Care

At Little Hearts at Home, we provide compassionate, specialized in-home care for children with complex medical needs through Georgia’s Pediatric Program (GAPP), a Medicaid-funded initiative. As an approved Medicaid provider, we offer both skilled and unskilled private home
care tailored to your child’s unique needs.
GAPP is designed to support children under 21 who meet medical necessity requirements,
ensuring they receive the care they need without delay. Unlike waiver programs, services can
begin immediately once your child is approved, giving families the support they need right when
it matters most.
Our intake and evaluation process takes up to 30 days, during which we carefully review your
child’s needs to create a customized service plan. Parents play an active role in this process,
partnering with our team to develop a plan that aligns with your child’s specific medical and
personal needs.

Who is Eligible

Minimum Age Requirement – Must be at least 18 years old.
Educational Background – High school diploma or equivalent (some agencies may require certified training).
Background Check – Must pass criminal background screening and reference verification.
Health & Fitness – Physically capable of assisting clients with mobility and daily living activities.
Training & Certification – Completion of basic caregiver training or relevant certifications such as CPR and First Aid.
